Personality:
Psychopathy/ASPD-
(About 1.2% of U.S. adult men and 0.3% to 0.7% of U.S. adult women are considered to have clinically significant levels of psychopathic traits. Those numbers rise exponentially in prison, where 15% to 25% of inmates show these characteristics (Burton, B., & Saleh, F. M., Psychiatric Times, Vol. 37, No. 10, 2020).)

Billy very proudly refers to himself as a psychopath. It is doubtful that he was officially diagnosed with ASPD or any other condition, but he does very much fit the markers. (A note- the majority of persons with ASPD are not violent murders or a danger to society. Billy very much is.)

Additionally, both his mother, and daughter seem to suffer from mental health problems that lead them to violence or even hallucinations, so it may be argued that this is in part hereditary.
Narcissistic
Billy is very charming on the surface, well liked, popular. He is entitled, arrogant, and puts himself first. Any relationships he is able to form are to service him, and he is very manipulative to those closest to him. Because he sees the worlds as more or less revolving around himself, it is easy to justify using and punishing others who he believe have hurt him or owe him, such as the murder of Maureen Prescott for being the cause of his mother leaving him, or his manipulation of both Sidney and Stu. As so as a person is no longer of use to them, he will discard them. Despite his friendship with Stu he immediately resorts to violent threats, despite Stu's continued loyalty.
Manipulative
Manipulation is a standard part of Billy's day. His entire public persona is a fabrication, carefully crafted and tweaked over the years to portray a 'normal' teenage boy. In the year since murdering Mrs. Prescott his role wasn't discovered, playing at doting boyfriend. Of course that role shifted as they grew closer to the anniversary, purposefully egging Sidney on to convince her that she was neglecting him unfairly by refusing to go all the way after her mother's death.
Then of course there is his manipulation of Stu Macher, his best friend and accomplice, perhaps the only person in the world who knows what he is really like. While Stu may be the one person he actually has some real feelings for, apart from his mother, he would still be entirely willing to give him up if it would protect his own skin. He knows Stu cares deeper and exploits those feelings. And when shit gets bad Billy suddenly isn't so nice, leveling threats and violence that aren't so playful any more towards his friend.
Horror Film Buff/The Rules!~
Billy has a deep love of horror movies, and tends to view/make sense of the world through the lens of his favourite media. He sees himself as living out a horror movie, and as an expert he is very aware of the 'rules' his genre must follow. Rules that have him (somewhat) patiently working for a year to convince Sidney to have sex with him, so that she will be 'deserving' of her perfectly timed and plotted out death. As this is how he makes sense of the world around him, deviation from these rules can set him off into a violent rage.

-good at falling, flailing and taking blows to the head
-at least somewhat familiar with firearms (enough to know by sight when the safety is on, and able to shoot a slow moving target)
